We investigated the antimicrobial susceptibility of 105 Neisseria gonorrhoeae strains isolated in Okinawa Prefecture, Japan, between 2020 and 2023. Nearly all isolates were non-susceptible to penicillin G and tetracycline, and approximately 60% remained non-susceptible to ciprofloxacin. A decreasing trend was observed in non-susceptibility to cefixime. Resistance to ≥3, ≥4, and 5 antimicrobial agents was observed in 41.0%, 22.9%, and 4.8% of strains, respectively. Although no ceftriaxone-resistant strains were detected, spectinomycin (SPCM)-resistant and intermediate-resistant isolates were identified for the first time in Okinawa. This emergence may be linked to increased travel following the COVID-19 pandemic. Caution is therefore advised when using SPCM for treatment. With international travel projected to increase, continued surveillance of N. gonorrhoeae susceptibility trends remains essential.

Key words: antimicrobial susceptibility; Neisseria gonorrhoeae; spectinomycin resistance
