諸外国において、HIV陽性men who have sex with menにおけるhuman papillomavirus(HPV)に関連した肛門管扁平上皮癌の増加が報告されている。その前癌病変であるanal intraepithelial neoplasia(AIN: 肛門管上皮内腫瘍)のスクリーニングと治療には、子宮頸癌の経験が応用され、エキスパート・オピニオンに基づき諸外国で広く実施されているが、HIV領域の主要ガイドラインにおいて推奨されておらず、わが国における経験は極めて限られる。AINスクリーニングと治療のアルゴリズムを、当院における経験を交えながら、その実際を解説し、国内での実施における課題を議論する。
An increase in the incidence of human papillomavirus (HPV)-associated squamous cell carcinoma of the anal canal among HIV-positive men who have sex with men has been widely reported. The current screening and treatment of precancerous lesions, including anal intraepithelial neoplasia (AIN), is based on the experience with cervical cancer and is widely performed primarily in developed countries based on expert opinion, but is not yet recommended in major HIV guidelines. Experiences in Japan are very limited. Here, I describe the AIN screening and treatment algorithm, including our experiences, and discuss issues to be solved for the algorithm implementation in Japan.
