2017年から2023年の期間、泌尿器科単科施設である、ていね泌尿器科・ていね駅前泌尿器科で診療した梅毒症例55例の臨床的検討を後方視的に行った。梅毒症例の年次推移では、2021年以降症例数が急激に増加していた。2例は他院でRPRが下がりきらず直前まで長期抗菌薬治療を行われていたが、陳旧性梅毒と診断し治療は行わず経過観察とした。それ以外の53例を早期活動性梅毒と診断し治療を行った。治療は原則としてAMPC内服治療を行っており、治癒確認率は86.8%であった。治療した53例は全て男性であったこと、陰茎局所の症状を訴えて来院し、1期梅毒と診断した症例が約85%と多くを占めていたことは、泌尿器科単科施設に受診する梅毒症例の特徴と考えられた。感染源は風俗店を利用したものと、行きずりの相手からのものとを合わせると約7割を占めており、今後も感染リスクについて啓蒙していく必要があると思われた。
We retrospectively investigated syphilis cases at the Teine Urological Clinic and Teine Ekimae Urological Clinic from 2017 to 2023. The number of patients has rapidly increased since 2021. Two cases were diagnosed with late-stage syphilis after receiving prolonged antibiotic therapy at another clinic due to persistent rapid plasma reagin positivity. A total of 53 patients were diagnosed with early syphilis. As a general rule, we administer oral amoxicillin therapy, which has a confirmed cure rate of 86.8%. It was observed that all treated cases were male, with approximately 85% presenting with symptoms localized to the penis and diagnosed with primary syphilis. This suggests a characteristic profile of syphilis cases presenting to urology specialty facilities. Notably, approximately 70% of the cases originated from either exposure at a brothel or from casual partners encountered on the same day, indicating the necessity of continuing to raise awareness and education on this issue in the future.
