島根県東部地区における男性尿道炎とその関連疾患の原因微生物を把握するため、当院で淋菌・クラミジアPCR検査を施行した症例について検討した。2019年1月～2022年8月に性感染症を疑った403名、平均年齢37.7歳、441検体を対象とした。10代の症例が13名で全体の3.2%と少なかった。淋菌性尿道炎は58例。その内、淋菌・クラミジア混合感染は17例にみられた。非淋菌性尿道炎の内、クラミジア性尿道炎は118例、非クラミジア性非淋菌性尿道炎を86例に認めた。淋菌咽頭感染を3例、クラミジア性精巣上体炎を4例検出した。パートナーがSTIを指摘された78名中35例がクラミジア陽性、1例が淋菌陽性であった。陽性例の内、半数以上の18例が無症候例で9例が初尿白血球数正常であった。初回治療失敗例は淋菌性尿道炎が15例、クラミジア性尿道炎で34例、非クラミジア性非淋菌性尿道炎では22例に認めた。
Between January 2019 and August 2022, 412 patients visited a clinic for sexually transmitted infections (STI). Samples were collected and analyzed for Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) using polymerase chain reaction. Thirteen cases (3.2%) involved teenage patients. NG urethritis was identified in 58 patients, including 17 (29.3%) with concurrent CT infection. Out of the rest of the patients with nonNG urethritis, 118 were CT, and 86 were nonCT. Three patients with NG pharyngeal infection and 4 with CT epididymitis were also identified. Out of 78 patients whose partners were diagnosed with STI, 35 were diagnosed with CT (44.9%) and 1 with NG (1.3%). In the positive cases, 18 were asymptomatic and 9 had normal urine voiding. Primary treatment failure was observed in 15 (25.9%) NG urethritis, 34 (28.8%) CT, and 22 (25.6%) nonCT as well as nonNG cases, respectively.
