淋菌感染症は日本の性感染症の疫学において重要な性感染症の一つである。今回我々は新たに開発されたイムノクロマトグラフィー法を原理とする淋菌抗原迅速検査キットであるラピッドエスピー≪淋菌≫（SBバイオサイエンス株式会社：現・住友ベークライト株式会社）（以下、本製品）の臨床性能評価を実施する機会を得た。淋菌感染が疑われた女性から得た391例の子宮頸管ぬぐい液を用いた検討では、同原理の既承認品に対する本製品の陽性一致率は84.9%、陰性一致率は94.1%、全体一致率は92.8%であった。核酸検査を標準検査とした場合、本製品の感度、特異度、正確度はそれぞれ67.7%、99.3%、91.8%であった。核酸増幅法を用いた比較では、陽性的中率96.9%で、プライマリ・ケアが必要な場面で、ルールインを目的とした補助診断ツールとして適していると考えられた。これらの結果より本製品は即日検査を希望する女性患者に対して臨床応用するに足る性能を有していると示唆された。
Gonorrhea is one of the most critical infectious diseases in Japan. We performed a clinical evaluation of the newly developed Neisseria gonorrhoeae antigen rapid test kit, RapidSP ≪Gonorrhoeae≫, which is based on an immunochromatographic method. The new kit, approved kit, and standard nucleic acid amplification test were used to examine 391 collected cervical specimens. The sensitivity, specificity, and accuracy of this new kit compared to the approved kit were 84.9%, 94.1%, and 92.8%, respectively. Compared to the standard nucleic acid amplification method, the sensitivity, specificity, and accuracy of the new kit were 67.7%, 99.3%, and 91.8%, respectively. The new kit can be used as a good diagnostic tool and first-hand choice for ruling in N. gonorrhoeae infection because its positive predictive value against the standard nucleic acid amplification method was 96.9%. These results suggest that the new kit can also be used for female patients who need a rapid diagnosis.
