子宮頸部腟の乳頭腫(コンジローマ)の原因となるHPV型の同定Detection of HPV genotype in the cervicovaginal papilloma

尖圭コンジローマにHPV16型などの高リスク型が検出されることがある。コンジローマ患者の外内性器のHPV感染型を調査した。外陰部肛門尖圭コンジローマを組織microdissection法で解析し15例すべてにHPV6、11型を検出した。14例(93%)の患者の子宮頸部腟細胞を調べるとHPV6、11型、53%に高リスクHPV型が陽性であった。子宮頸部腟乳頭腫と診断された50例中47例(94%)の乳頭腫組織にHPV6、11型、3例にHPV71、81、89型が検出されたが、この3例は病理組織学的に乳頭腫ではなかった。

外陰部肛門尖圭コンジローマと子宮頸部腟乳頭腫にはHPV6、11型のみ検出された。外陰部肛門尖圭コンジローマ患者の約9割の腟内にHPV6、11型、約半数に高リスク型HPV感染があった。外陰部肛門尖圭コンジローマ患者は腟内の精査と子宮頸がん細胞診が必要かもしれない。

Condyloma acuminatum, which develops on the genital skin or mucosal epithelium, is caused by human papilloma virus (HPV) infection. Although the predominant causative agents are HPV types 6 and 11, high-risk types including HPV16 are occasionally identified in these lesions.

The present study conventionally defined condyloma accuminatum of the anus and the vulva as “anogenital condyloma” and the condyloma in the cervix and vagina as “cervicovaginal papilloma.” The latter lesion is sometimes difficult to identify compared to the former because the lesion is flat and obscure. The presence of hairpin-like vessels in the papillary projections is a typical finding in cervicovaginal papillomas. We investigated the associations between anogenital condyloma, cervicovaginal papilloma, and squamous intraepithelial lesion (SIL) and detected the related HPV genotypes. All anogenital condyloma tissues were positive HPV6 or 11. The cervicovaginal cell specimens of these patients were positive for HPV6 and 11 (93%) and high-risk (HR) HPV types including HPV16 (53%). Among 50 patients with cervicovaginal papilloma, the tissues of 47 (94%) were positive for HPV6 or 11, and those of three patients (6%) were positive for HPV71, 81, and 89. However, a histological analysis of the lesions of the latter three cases showed that these were neither condylomas nor papillomas, suggesting that anogenital condyloma and cervicovaginal papilloma are induced exclusively by HPV types 6 or 11. Among patients with cervicovaginal papilloma, 65% were complicated with cervicovaginal SIL, and 42% were positive for HR-HPV types. Therefore, patients with condyloma might be at a high risk for cervical cancer in the future and should have the screening test.
