The first-line treatment for Pneumocystis jirovecii pneumonia (PCP) is trimethoprim-sulfamethoxazole, while the second-line treatment is pentamidine. Both medications are essential for managing PCP. However, they have various side effects, some of which can be fatal. Although pentamidine-induced anaphylaxis is reported to occur infrequently, it is an adverse effect that requires prompt treatment when it develops. Here, we present a case of anaphylaxis caused by pentamidine used for the treatment of PCP.
