梅毒患者の受診行動と診断経緯に関する検討Consideration of the consultation behavior and diagnosis process for syphilis patients

最近の梅毒患者の受診行動と診断経緯を検討して、より早期の段階で梅毒を発見・治療するために必要な診断技能について考察した。梅毒を疑った37例中梅毒と診断したのは25例(1期16例、2期6例、無症候性梅毒3例)、除外例12例であった。PCRを施行した23例中陽性は17例(73.9%)であり、とくに1期では従来の技法では診断が不可能と思われるような、ごく早期の微小潰瘍や、痂皮化した潰瘍病変、あるいは血清学的検査が陽性化する前の症例でも診断することができた。一方で2期梅毒疹や正常皮膚に覆われた初期硬結からは検出できなかったので、広くPCRを普及させるためには手技や適応選択の普遍化が必要である。また、検討に用いた梅毒抗体の自動化法においてはTPLAがRPRよりも早く立ち上がることが示され、早期梅毒の診断の一助となり得ると考えられた。

In this study, we discuss the diagnostic skills necessary for the diagnosis of syphilis and provide treatment at an earlier stage by considering the behavior of recent syphilis patients and the diagnosis process. Twenty-five syphilis cases were diagnosed (16 primary cases, 6 secondary cases, and 3 cases of asymptomatic syphilis) after excluding 12 cases from among the potential 37 cases. PCR testing was performed for 23 patients, and 17 (73.9%) tested positive. In particular, among primary syphilis patients, we could diagnose patients whose diagnoses would not have been possible using conventional techniques, such as those involving a small ulcer at a very early stage, those with crusted ulcerated lesions, and those who were not identified as syphilis positive with serological testing. However, we could not obtain definitive results for syphilis diagnosis using PCR for secondary cases with syphilitic lesions and initial sclerosis hidden by normal skin. Therefore, in order to increase the use of PCR, it is necessary to standardize these techniques and application methods. Moreover, with regard to the automation of testing using syphilitic antibodies, TPLA is demonstrated to become available faster than rapid plasma reagin and is believed to be useful for the early diagnosis of syphilis.
