梅毒無料匿名検査陽性例におけるRPRカード法と自動化法の定量値の比較Comparison between quantitative values obtained using RPR card and automated RPR tests for syphilis in free-of-charge, anonymous examinations

梅毒無料匿名検査の血清検体7,273例で、Rapid Plasma Reagin test(RPR)のカード法または自動化法検査法で陽性を示した92例について、血清希釈倍数や希釈倍率から求めたそれぞれの定量値を比較した。80例は両検査法とも陽性を示したが、うち8例(10.0%)については16倍および16.0 R.U.の届出基準値を一方が超え、一方が下回った(うち7例はRPRカード法のみ届出基準値以上)。また、キットの判定基準(陽性または陰性)で不一致がみられた12例のうち、11例はRPRカード法のみ陽性、1例は自動化法のみ陽性であった。近年、梅毒の届出数は急増しているが、自動化法の届出基準の追加による梅毒届出数の増加への影響は小さいものと考えられる。

For sexually transmitted infections (STI) control measure, comparative studies were conducted for RPR card and RPR automatic test results for syphilis using 7,273 blood specimens obtained from free-of-charge, anonymous STI examinations at Tokyo Metropolitan Institute of Public Health. Cases (n=92) that tested positive for syphilis using one or both tests were compared for quantitiative values calucated using the blood sample dilution ratios. Among the 80 cases that tested positive in both methods, eight (10.0%) demonstrated discrepancy; the results were higher in one test than the reference value of syphilis reporting in Japan, but in the other test, they were lower. The remaining 12 cases showed positive results using one method and negative results in the other. The card test provided positive results only for 11 cases. Recently, the number of syphilis cases has rapidly increased in Tokyo, and the influence of additional border for the RPR automatic test on this rapid increase is relatively low.
