In children, condyloma acuminatum is relatively rare, and the pain associated with the surgical treatment may be intolerable for them. Since the efficacy and safety of topical imiquimod remain unknown, its pediatric use is presently prohibited. Therefore, we used topical podophyllin to treat condyloma acuminatum in two children. Two girls aged 22 and 23 months were referred to our clinic with a diagnosis of condyloma acuminatum. A 20% ethanol solution was created, using podophyllin from Sigma-Aldrich, and applied to the papules twice a week. The lesions gradually regressed and the pain and inflammation became minimal. Finally, after four and nine months, respectively, both cases healed completely without subsequent recurrence. In Japan, imiquimod is the only topical drug used to treat condyloma acuminatum. However, children do not have access to it through the Japanese health insurance system. After familial consent, we used podophyllin combined with a reagent and achieved good results. Although there are no data presently available on the safety of podophyllin in children, it is considered as one of the first choices for the treatment of condyloma acuminatum in children. In other countries condyloma acuminatum is treated with podophylox, which is considered to be safer than podophyllin. Introduction of podophylox is required in Japan.
